Audiology Benefit
The
Audiology Benefit is provided to the member only and
exclusively at the
The Audiology Benefit consists of a comprehensive audiologic evaluation that determines the type and degree
of the hearing loss which is outlined on an audiogram. If the evaluation
confirms a hearing deficiency, the member will be given the audiogram and
report to take to an Ear, Nose and Throat Specialist for either medical
treatment or medical hearing aid clearance. The member must apply directly to
his/her basic health insurance carrier for reimbursement for the Specialist’s
fee; it is not covered by the Audiology Benefit.
The Center’s Audiologist will review the Specialist’s report and determine if a
hearing aid is required. If a hearing aid is required, the Plan will provide
one at no cost to the member. If a second hearing aid is necessary, it will be
provided at the Plan’s invoice cost. The member will be evaluated and fitted
for a hearing aid. Once the hearing aid is dispensed, the member can return for
an adjustment to ensure that the aid fits well and is functioning properly for
their individual needs.
The Health & Security Plan will provide a maximum of one hearing aid and
one mold (if applicable) within a three year period. Thereafter, the member can
be reevaluated and, if deemed necessary by the Center’s Audiologist, a
replacement may be provided.
Replacement batteries are not provided. Aural rehabilitation is not provided by
the Plan.
If your basic health insurance coverage provides reimbursement for an audiology benefit, you will be required to sign an
authorization form allowing the Health Options Medical P.C. to file a claim
with your insurance carrier.
